Living in Japan Cost

How much does childbirth cost in Japan, and what does insurance pay?

Last checked: 2026-11-06

If you are on public health insurance in Japan, your insurer pays in principle 500,000 yen per child when you give birth, and the average cost was about 520,000 yen in fiscal 2024. You pay the difference, plus any private room and extras. The allowance comes through your insurance, so being enrolled is what matters. A new system is planned, but its amounts are not set yet.

What you get

Support payment, second part50,000 yen x the number of children expectedYour municipality

Multiple births are paid per child. The support payment needs an application at the municipality where you have your resident record. You apply for the first part after a doctor confirms the fetal heartbeat, and for the second part from 8 weeks before the due date. It comes with a meeting at the municipal office, and some municipalities offer coupons instead of cash.

What it costs

MHLW's chart shows the average cost at all facilities rising to about 507,000 yen in fiscal 2023 and about 520,000 yen in fiscal 2024. This leaves out a private room and the compensation scheme fee, among other items. The ministry's site Shussan Navi lists fees by facility.

Direct payment: worked examples

Under direct payment, the hospital claims the allowance from your insurer, so you pay only the rest at the counter. Not every facility offers this, so ask. These bills are our own examples.

You can also refuse direct payment, pay the hospital yourself and claim the allowance later.

What is changing

A law promulgated on June 5, 2026 changes childbirth benefits. MHLW's plan has three parts:

The law says this starts within 2 years of promulgation, on a date set later. MHLW's October 2026 paper planned to settle the amounts around December 2026. So the 500,000 yen allowance is still the rule for now. Check again before your due date.

Which insurance pays

After the birth: foreign parents

Yokohama gives these steps. Offices and details may differ in your city.

  1. Register the birth at the family register counter in your municipality.
  2. Status of residence. If both parents are non-Japanese, the child has no status of residence for 60 days from birth. To stay longer than 60 days, apply to immigration within 30 days of the birth.
  3. Health insurance. Enroll your baby as soon as possible.
  4. Child allowance. Apply within 15 days. See our child allowance guide.
